A very well known social services organization is looking for a Registered Nurse - Case Management to join their growing team in the Bronx, NY area. 

Job Function:

• Day Shift
\n
• Monday - Friday, 9am - 5pm
\n
• Provides direct and supportive healthcare to individuals that reside in the client's residential programs.
\n
• The position focuses on coordinator of care/case management for individuals and staff training oversight for the provision of care.
\n
• This person will also be responsible for documenting individual's status, health needs and follow-up.
\n

Job Requirements:

\n

• Bachelor's degree or Master's degree in Nursing
\n
• 1 year of professional experience in delivering health care to people with disabilities
\n
• NYS Certification is mandatory
\n
